2.64.500 Application for Benefits <br /> A. Applications for benefits to be paid under this chapter shall be made upon <br /> forms provided by the board of trustees, and shall contain full information, <br /> from which the board may determine the eligibility of the applicant. If an <br /> application for retirement is approved, no further compensation for <br /> services shall thereafter be paid by the city to such employee except for <br /> such part-time service, not exceeding one thousand one hundred (1,100) <br /> hours per year, to the city. <br /> B. Applications for distributions shall be made in accordance with rules and <br /> procedures established, and upon forms (or such other medium) provided, <br /> by the board of trustees or, if authorized by the board, the manager of the <br /> investment funds. <br /> 2.64.510 Reserved <br /> 2.64.515 Optional Form of Pension <br /> If the designated beneficiary of a member is the spouse of a member, the member may <br /> elect prior to the member's normal or early retirement date, to receive, in lieu of the <br /> member's regular normal or early retirement pension, a reduced pension payable for so <br /> long as the member shall live, with such reduced pension continuing to the member's <br /> spouse for his or her lifetime following the member's death. Such reduced pension shall <br /> be the actuarial equivalent of the pension regularly payable. <br /> 2.64.520 Required Minimum Distributions—Time and Manner of Distribution <br /> A. Required Beginning Date: The member's entire interest will be <br /> distributed, or begin to be distributed, to the member no later than the <br /> member's required beginning date. <br /> B. Death of Member Before Distributions Begin: If the member dies before <br /> distributions begin, his or her entire interest will be distributed, or begin to <br /> be distributed, no later than as follows: <br /> 1. If the member's surviving spouse is the member's sole designated <br /> beneficiary, then subject to section 2.64.520.B.5 distributions to <br /> the surviving spouse will begin by December 31 of the calendar <br /> year immediately following the calendar year in which the member <br /> died, or by December 31 of the calendar year in which the member <br /> would have attained age 70%2, if later. <br /> 2.
