(b) Commercial General Liability Insurance on an occurrence basis with <br />limits of liability not less than $1,000,000 per occurrence and $2,000,000 in the aggregate for <br />Personal Injury, Bodily Injury and Property Damage. Coverage shall include the tbllowing <br />extensions: <br /> <br />(1) <br />(2) <br />(3) <br />(4) <br />(5) <br /> <br />Contractual Liability <br />Products and Completed Operations <br />Independent Contractors Coverage <br />Broad Form General Liability Extensions or equivalent <br />Coverage for X, C and U Hazards <br /> <br /> (c) Motor Vehicle Liability Coverage for all vehicles used in the performing <br />of the contract. Limits of Liability shall not be less than $1,000,000 per occurrence and <br />$2,000,000 in the aggregate for Bodily Injury and Property Damage. <br /> <br /> (d) Additional insured. Commercial General Liability Insurance as described <br />above shall include an endorsement stating that the City, iucluding all elected and appointed <br />officials, all employees and volunteers, all boards, conunissions and/or authorities and their <br />board members, employees and volunteers, shall be an additional insured. <br /> <br /> (e) Cancellation Notice. Workers' Compensation Insurance, Commercial <br />General Liability h~.suxance, and Motor Vehicle Liability Insurance as described above shall <br />include an endorsement stating that thirty (30) days advance written notice of cancellation, <br />nonrenewal, reduction and/or material change shall be sent to: <br /> <br />City Mmmger <br />City of University City <br />6801 Delmar Boulevard <br />University City, MO 63130 <br /> <br /> (f) Proof of Insurance. If so requested by the City, Metromedia and any of its <br />contractors hired by Metromedia shall within thirty (30) days of such request provide the City <br />proof of the insurance required within this section. <br /> <br /> 17. Performance Bond.
