B. Applications for distributions under Section 2.62.407 shall be made in <br /> accordance with rules and procedures established, and upon forms (or <br /> such other medium) provided, by the board of trustees or, if authorized by <br /> the board, the manager of the investment funds. <br /> 2.62.510 Reserved <br /> 2.62.515 Reserved <br /> 2.62.520 Required Minimum Distributions—Time and Manner of Distribution g` <br /> A. Required Beginning Date: The member's entire interest will be <br /> distributed, or begin to be distributed, to the member no later than the <br /> member's required beginning date. <br /> B. Death of Member Before Distributions Begin: If the member dies before <br /> distributions begin, his or her entire interest will be distributed, or begin to <br /> be distributed, no later than as follows: <br /> 1. If the member's surviving spouse is the member's sole designated <br /> beneficiary, then subject to section 2.62.520.B.5 distributions to <br /> the surviving spouse will begin by December 31 of the calendar <br /> year immediately following the calendar year in which the member <br /> died, or by December 31 of the calendar year in which the member <br /> would have attained age 70'/2, if later. <br /> 2. If the member's surviving spouse is not the member's sole <br /> designated beneficiary, then subject to section 2.62.520.B.5 below <br /> distributions to the designated beneficiary will begin by December <br /> 31 of the calendar year immediately following the calendar year in <br /> which the member died. <br /> 3. If there is no designated beneficiary as of September 30 of the year <br /> following the year of the member's death, the member's entire <br /> interest will be distributed by December 31 of the calendar year <br /> containing the fifth anniversary of the member's death. <br /> 4. If the member's surviving spouse is the member's sole designated <br /> beneficiary and the surviving spouse dies after the member but <br /> before distributions to the surviving spouse begin, this section, <br /> other than section 2.62.520.B.1, will apply as if the surviving <br /> spouse were the member. <br /> 5. If the member dies before distributions begin and there is a <br /> designated beneficiary, distribution to the designated beneficiary is <br /> not required to begin by the date specified in sections 2.62.520.B.1 <br /> or .2 if the member's entire interest is distributed to the designated <br />
