Ê×òÜÛÚ×Ò×Ì×ÑÒÍ <br />Membership fees that are paid by electronic funds transfer (EFT) will incur a two month <br />membership fee as a penalty if the cancellation is granted within the first year. After one year of <br />EFT payments a member may cancel without penalty with 30 days notice. <br />Members’ monthly payments must be current in order to request a cancellation. <br />Membership fees that have been paid upfront will incur a 25% penalty if the cancellation is <br />granted. No penalty will be accessed in the case of death. <br />Cancellation will be granted when the member is deceased. A copy of the death certificate is <br />required within 90 days of the date of the certificate. Individual membership refunds will be made <br />to the family of the deceased. Senior couple membership will be refunded the difference in the <br />membership fee for senior couple and senior individual. Family membership will only be refunded <br />if the membership included only 2 persons and will be refunded the difference in the membership <br />fee for the surviving individual. All cancellations because of death will be prorated based on time <br />left on the membership. The refund will be given to the listed emergency contact. If no <br />emergency contact is listed any family member may make the request. <br />Requests for membership cancellation due to medical conditions will be granted only with <br />documentation from a licensed physician that the membership cannot be fulfilled due to a medical <br />condition. <br />Requests for membership suspension due to medical conditions will be granted only with <br />documentation from a licensed physician that the membership cannot be fulfilled for a <br />specified time period due to a medical condition. The member understands they are <br />obligated to continue the membership upon their return and their extension is only for the <br />remaining period of the membership. <br />Requests for membership cancellation due to military service will be granted only with <br />documentation of a letter of deployment for military service. <br />Requests for membership cancellation due to relocation will be granted only with documentation <br />for relocation at least 15 miles away from current address. <br />Requests for membership suspension due to relocation will be granted only with <br />documentation for relocation at least fifteen (15) miles away from current address for a <br />specific time period. The member understands they are obligated to continue the <br />membership upon their return and their extension is only for the remaining period of the <br />membership. <br />Requests for membership cancellations due to financial hardship will be granted only with <br />evidence of loss of employment. <br />or suspensions <br />Requests for membership cancellations for any of the above reasons or other <br />special circumstances will be reviewed and the Department Director has the discretion to make <br />exceptions. <br />For purposes of this policy memberships are non-refundable. <br /> <br /> <br />
