Ê××òÎÛÍÐÑÒÍ×Þ×ÔÌ×ÛÍ <br />suspension <br />Cancellation and requests will be reviewed, approved or denied by the Department <br />Director. <br />Ê×××òÐÎÑÝÛÜËÎÛÍ <br />or suspension <br />Members requesting cancellation of membership contracts must submit their <br />or suspended <br />written request at least thirty days prior to the date to be cancelled . <br />or suspension <br />Documentation for cause of cancellation must be submitted with the request to <br />the Department Director for approval.
