October 19, 2005 Plan Commission Minutes Page 4 of 16 <br />(5) Description of proposed design for screening the facility from public view, <br />including plans for installation and maintenance of landscaping, sample exterior <br />materials and colors. Where applicable, the plan shall show existing surrounding <br />landscaping, proposed landscaping, and a landscape maintenance plan during the <br />construction and operation of the facility;. <br /> <br />(6) Description of the proposed wireless communications facility and equipment, <br />including but not limited to manufacturer(s), power output, frequency range, <br />equipment shelter(s) and dimensions. <br /> <br /> (7) Description of the lighting scheme if required by the FAA or other <br />state or federal agency; <br /> <br />(8) A description of the proposed security measures to protect against unauthorized <br />access; <br /> <br />(9) A visual analysis to assess the views of the proposed facility from public areas <br />and private properties; <br />(10) Decommissioning and Repair Bond: The applicant shall submit a statement <br />that it will provide a surety bond in accordance with Subsection 2j;. <br />(11) Indemnification. The applicant shall submit a statement of <br />indemnification in accordance with Subsection 2k;. <br />(12) Insurance: The applicant shall submit a statement of insurance in <br />accordance with Section 2l;. <br />(13) Multi-municipality Commission Procedure. If the City has agreed to <br />participate in a multi-municipality commission to coordinate new tower or <br />structure applications, an application herein for a conditional use permit shall be <br />submitted to such commission simultaneous with the filing of the request with <br />the city. The Plan Commission may consider any comments from such multi-city <br />commission, but shall not allow delay in receiving such comments to <br />significantly delay any decision on the application.; <br /> <br />(14) Network Compatibility. The applicant should demonstrate how the <br />proposed site fits into the overall network of the low- power mobile radio service <br /> system within the City and adjacent cities. <br /> <br /> (15) Demonstration of Need.
