These reports shall be certified as correct by an authorized officer of the Company and there shall be submitted along with them such other reasonable information as the City shall request with respect to the Company's properties and expenses related to its CATV System operation within the City. 2. An "Annual Rep~t of Cable Television Systems" (FCC Form 325, Schedules 1, 2, 3 and 4) or the appropriate successor forms. 3. An annual total facilities report setting forth the physical miles of plant constructed, rebuilt or in oper- ation during the fiscal year. Such report shall also contain any revisions to the network "as-built" maps filed with the City. 4. The Company schedule of charges, application forms for regular subscriber service, policy regarding the process- ing of subscriber~ complaints, delinquent subscriber disconnect and reconnect procedure and any other terms and conditions 9dopted as the Company's policy in connection with its subscribers shall be filed with the City and con- spicuously posted in .the Cb~pgny's local office. All such terms and conditions, including schedule of charges, must be approved by the City prior to their becoming effective. 5. The performance bond or a certified copy thereof and written evidence of paymen~ of required premium, as applicable. 6. The letter of credit or a certified copy thereof and written evidence of payment of required premium. 7. All policies of insurance qr certified copies thereof and written notice of payment of required premium. 8. An Ownership Report, indicating all persons who at any time during the preceding year did control or benefit from an interest in the franchise of ten percent (10%) or more. The Company shall notify the City of all changes in owner- ship affecting ten percent (10%) or more within thirty (30) days of the change.. 9. All rules, regulations, terms and conditions which the Company has adopted for conduct of its business. 10. One (1) Copy of a report on the network's technical measurements as set forth herein. b) REGULATORY REPORTS REQUIRED All petitions, applications and communications of all types submitted by the Company to the Federal Communi- · cation Commission, Securities and Exchange ..Commission, or any other Federal or State regulatory commission or agency having Jurisdiction over any matter affecting operation of the Company's network shall be submitted simultaneously to the City. C) RECORDS REQUIRED The Company shall at all times maintain: 1. A record of all complaints received and interruptions or degradation of service experienced for the preceding three (3) years. 29
