Exhibit B <br /> <br />REVISION TO CABLE TELEVISION CONSUMER PROTECTION AND CUSTOMER <br />SERVICE REGUIJATIONS <br /> <br />Paragraph b(2) is amended by deleting the word "quarterly' and inserting therein the word "monthly". <br /> <br />Paragraph j is ameaded by renumbering the paragraph as k. <br /> <br />New paragraph j is added to read as follows: <br /> <br /> "(1) If disputes or disagreements between the subscriber and the Franchisee are not able <br />to be resolved between said parties, the subscriber may then complete a complaint form, as approved <br />by the City, and file it with the City. A copy shall promptly be mailed to the Franchisee. Within ten <br />(10) working days both subscriber and Franchisee will be mailed notification indicating the date, time, <br />and place the complaint will be heard by the City Manager or his/her designee. <br /> <br /> (2) During the hearing both complainant and Franchisee will be allowed to present <br />information regarding said dispute. <br /> <br /> (3) At~ter the hearing, the City Manager or designee will have ten (10) working days to <br />render a decision and notify both the Franchisee and the complainant. <br /> <br /> (4) The Franchisee must display and provide the complaint forms at its Customer Ser¥ice <br />Center. In additioa, Franchisee shall advise any complaining subscriber of his/her fight to file a written <br />complaint." <br /> <br />Paragraph k is renumbered as I and the figure "$100.00" is amended to read "$500.00". <br /> <br /> <br />
