Session 1758 <br />May 24, 1999 <br /> <br />#2 - TCI FCC Forr~s 1240-1205: Mr. Ollendorff, on behalf of Mr. Gregory Rose, Assistant <br />City Manager, recc,mmends acceptance of TCI's FCC Forms 1240 and 1205, representing <br />its annual filing fo~' adjustments to the basic service, equipment, and installation prices. <br />Staff has completed its review of both forms and everything appears to be in order and <br />consistent with fe(leral requirements. Approval of the forms will result in the following: a <br />15% reduction in :he cost for installations; equipment rates will remain about the same; <br />and a 7% increase in the cost for basic cable. <br /> <br />#3 - 1999-2000 Curb & Sidewalk Replacement Contract: The tabulation of bid proposals <br />for the above nam,.~d project are as follows: <br /> <br />D. I recommend that this project be <br />awarded to D. Bernish Construction Co., Inc. for their low bid of $330,679.25. Of this <br />figure, about $14, )00 worth of sidewalk work will be done in University Heights #1 <br />Subdivision which will be reimbursed to the City. <br /> <br />#4 - 1999-2000 ~nnual Order- Janitorial Supplies: Bids for furnishing the City's <br />estimated requirenlents for industrial supplies for the 1999-2000 fiscal year have been <br />received. These suaplies include various janitorial and maintenance chemicals and paper <br />products. The City has been obtaining numerous items (58 items) on a Sole Source Basis <br />from National Sanitary Supply since 1996. National Sanitary was merged with Unisource <br />during the last yea'. The Purchasing Specialist felt it was necessary to obtain competitive <br />pricing for those ib~ms previously sole-sourced to National Sanitary.
