(ii) The day or days for access cablecasting <br /> desired~ <br /> <br />(iii)The time segment (i.e., 9~00-10~00 a.m., <br /> etc.)~ <br /> <br />(iv) <br /> <br />The nature of the program, i.e. video-tape, <br />live, etc. If v~deo-tape, a statement of <br />the type and size, i.e., 3/4 inch video <br />cassette. <br /> <br />(v) A full and complete statement of the topic <br /> or topics to be d~scussed or portrayed~ <br /> <br />(vi) <br /> <br />Whether the applicant desires use of <br />available cablecasting equipment and faci- <br />lities~ the time and duration for which use <br />of the equipment is requested; <br /> <br />(vii)A statement that the access channel user is <br /> familiar with the definitions and operating <br /> rules of the access channel involved. <br /> <br />(viii)A statement that no advertising mater~al is <br /> to be cablecast~ <br /> <br />(ix) <br /> <br />A statement that no lottery is involved and <br />that no lottery information is to be cable- <br />cast, except as permitted by Section 1 (d) <br />and 2 (c). If lottery advertisements, <br />lists, prizes or information are to be <br />cablecast, a complete description of how <br />such a cablecast is consistent with Section <br />1 (d) and 2 (c) must be made; <br /> <br />(x) A statement that no obscene or indecent <br /> material is to be cablecast; <br /> <br />(xi) <br /> <br />A statement that all appropriate arrange- <br />ments and clearances have been obtained <br />from broadcast stations, networks, <br />sponsors, music licensing organizations, <br />performers' representatives and without <br />limitation by the above, any and all other <br />person (natural or otherwise) as may be <br />necessary for authorization to transmit its <br />program material over the facilities of the <br />Company; <br /> <br />(xii)Tbe production facilities and personnel <br /> which will be necessary for presentation of <br /> the access cablecast, if any. <br /> <br />(b) All requests for access are to be submitted at least <br />seven (7) days in advance of the desired date and time <br />of the proposed access cablecast. <br /> <br />(c) Requests for use of public access time on behalf of <br />minors (as defined by applicable state laws) should be <br />made in the name of a responsible adult. Any such <br />minors using the equipment of the Company shall be <br />accompanied by a responsible adult who shall supervise <br />the use of the facilities and equipment and execute the <br />necessary assurances that authorization has been <br /> <br />42 <br /> <br /> <br />
